Complete Decongestive Therapy (CDT) is considered the gold standard treatment for Lymphedema at this time. CDT is recommended as the preferred treatment intervention by the leading organizations in the field of lymphedema including the Lymphology Association of North America (LANA), the National Lymphedema Network (NLN), and the International Society of Lymphology (ISL).
CDT is a two-phase program consisting of a treatment phase and a maintenance phase. During the treatment phase, treatment is performed daily for two to four weeks. The maintenance phase begins upon completion of the formal treatment phase and consists of daily self-care and planned reassessments with a LANA Certified Lymphedema Therapist.
The treatment phase of CDT is a four-component program:
- Skin and Nail Care: Skin and nail care consists of visual and manual inspection of the limb, instruction in arm or leg care guidelines, and moisturizing of the limb.
- Manual Lymph Drainage (MLD): Manual Lymph Drainage (MLD) is a manual technique, similar to massage, that is used to facilitate lymph flow in the limb and at regional areas of the trunk.
- Compression Bandaging: Compression bandaging consists of a multi-layer bandage that is applied over the entire limb. This bandaging includes foam padding and short stretch bandages which facilitate lymph flow. The compression bandage is worn 24 hours per day during the treatment phase.
- Therapeutic Exercise: Mild therapeutic exercise is performed with the bandaging in place to facilitate the lymphatic flow through the muscle-joint pump mechanism of the involved limb.
The treatment phase is performed daily (5 times per week) for two to four weeks. Each treatment session is approximately one hour in duration. Perometer(R) reassessment of girth and volume measurements are performed at the end of each week. Once measurements plateau, the maintenance phase begins.
The maintenance phase of CDT is a self-care program that is performed independently with the goal of maintaining the clinical results from formal treatment. The maintenance phase consists of:
- Skin and Nail Care
- Self-MLD
- Compression Garments
- Home Exercise Program
- Self-Compression Bandaging
Upon completion of your CDT treatment, a compression garment is recommended to maintain the reduction of limb size and volume achieved through treatment intervention. For your convenience, we offer both off-the-shelf and custom-fit compression garments.
Regularly scheduled Perometer® reassessments of limb girth and volume are recommended upon completion of formal treatment intervention in order to indentify any exacerbation of Lymphedema at it's earliest stage. Our treatment program recommends that Perometer® reassessments be performed every three months for the first two years following formal cancer treatment, then performed once every six months for active, diligent monitoring and management of your Lymphedema for a lifetime.
